Rockin Rewards Club

If you are an existing patient of Dr. Ralph, you may participate in our rewards club! You will receive a card that we will punch (one punch per visit) when the rules are followed below. Cards are not transferable between patients. Emergency visits do not apply. Follow the simple rules below:

1. Teeth must be clean and gums must be in healthy condition.
2. No loose bands or brackets since the last regular visit.
3. No broken appliances or broken wires since the last visit.
4. All appliances and elastics must be worn as prescribed by Dr. Ralph.
5. No missed appointments or excessive time since your last regular visit.

You may redeem your cards after you have earned your punches for the prize equivalent to the punches received. See the display in the waiting room for actual rewards available. You can save up your cards for higher prizes. If you lose your card you can get a new one, but punches may not be replaced.

REWARD CARDS MUST BE PRESENT AT APPOINTMENT TO RECEIVE PUNCHES.
